Aneurysmal bone cysts (ABCs) are vascular local proliferative, non-neoplastic, benign lesions seen in children and young adults. 75% of aneurysmal bone cysts occur before the age of twenty. The cyst often develops in long bones and makes up 1.4% of primary bone tumors. Spinal aneurysmal bone cyst is rare and commonly seen in the thoracic and cervical regions. The posterior elements of a single vertebra are usually involved in the spinal region. The lamina, pedicle and facet joints are more commonly affected and the lesion may also expand towards the corpus. We present the successful total excision of a lumbar spinal giant aneurysmal bone cyst by combining the surgery with pre-operative coil embolization. Pre-operative arterial embolization contributes to decreased morbidity and mortality rates by reducing the surgery duration and intraoperative hemorrhage.
