A patient harboring a distal posterior cerebral artery aneurysm is presented. The aneurysm located on the right calcarine artery was totally excised. So far only two cases with such a distal aneurysm of PCA have been published. This very rare anatomic site for an aneurysm and the related literature are reviewed.
