MOTION-PRESERVING SURGERY FOR LUMBAR SPINAL STENOSIS: LONG-TERM CLINICAL AND RADIOLOGICAL OUTCOMES OF POSTERIOR DYNAMIC STABILIZATION IN A LARGE COHORT OF 307 PATIENTS

Abstract

Aim
Lumbar spinal stenosis is a degenerative spinal disorder characterized by neurogenic claudication and functional impairment. The aim of this study was to evaluate the clinical and radiological outcomes of decompression combined with a semi-rigid posterior dynamic rod stabilization system in this patient population.

Material and Methods
A total of 307 patients who underwent decompression combined with a semi-rigid posterior dynamic rod stabilization system were retrospectively analyzed. Clinical outcomes were assessed using preoperative and postoperative pain scores and neurological symptom evaluation. Radiological parameters and postoperative complications, including adjacent segment disease (ASD), were evaluated during mid-term follow-up. Statistical analyses were performed using IBM SPSS Statistics version 24.0. Continuous variables were expressed as mean ± standard deviation or median (minimum–maximum), and categorical variables as frequency and percentage. Comparisons were performed using the paired samples t-test or Wilcoxon signed-rank test. Categorical variables were analyzed using the chi-square or Fisher’s exact test. Logistic regression analysis was used to identify factors associated with ASD.

Results
Significant postoperative improvements were observed in pain scores and neurological symptoms (p < 0.05). Radiological evaluation demonstrated preserved segmental alignment and adequate stabilization. The complication rate was low, and the incidence of ASD was acceptable. Advanced age, higher Pfirrmann grade, and a greater number of operated levels were associated with ASD.

Conclusion
Decompression combined with a semi-rigid posterior dynamic rod stabilization system provides favorable clinical and radiological outcomes in selected patients with lumbar spinal stenosis while preserving controlled segmental motion.
